Risk of institutional care

Can the situation be returned to the same as before? Does the customer have a new illness or medication?
- The role of family members
Can the client’s family members take a bigger role in caring for the client?
- Temporary services
Can the situation improve if the customer’s services are temporarily increased?
- Preventive measures
Can physiotherapy, new medication or other measures improve the situation?
- Care home
If the client is no longer able to live at home, initiate the steps to transition to care home.
