Falls
Alarm level explanation: The customer’s risk of falling is high. The customer may not have fallen, but the risk of falling is elevated. Find out what contributes to falls or increases the risk of falling.
- Movement or impaired balance
Ensure movement, update the home rehabilitation plan, guide the use of a mobility aid.
- The aid is unsuitable or missing
Get the necessary aid.
- Living environment, walkways, thresholds, carpets, footwear, lighting, stairs, stair railings and heat
Correct the defects that can be corrected immediately. Notify the customer or his case manager of the need for follow-up measures (cleaning, purchase of footwear, need for repairs).
- More than 5 medications or medications that increase the risk of falls in use > 2 (tranquilizers, mood, epilepsy, Parkinson’s, fluid removal, blood pressure, sleeping pills)
Optimization of medication together with a physician or with a pharmacist.
- Disease status, acute infections, blood pressure fluctuations, orthostatism, Parkinson’s disease, AVH, memory disorder, impaired vision, dizziness
Consult a physician for the treatment of the disease.
- Nutrition, BMI < 25, unintentional weight loss (>3 kg), reduced food intake (vomiting, decreased appetite, digestion/chewing/swallowing problems), dehydration, alcohol use
Enhancement of nutrition/hydration, use of supplements (protein and energy products), meal service, eating assistance.
