- Check for signs of acute dehydration
Is the pulse or blood pressure higher than before or is the client confused?
- Check for signs of infection
Does the client have fever, cough, diarrhea, vomiting or unusual fatigue?
- Medication
Does the client use diuretics or other medication that can affect fluid balance? If needed, contact a physician and order the necessary laboratory tests.
- Physical and cognitive barriers
Make drinks available and ensure the client remembers to drink.
