More info: The stability of the client’s health status has deteriorated. If health has declined recently, identify the cause of the change and assess whether there are measures that could restore the situation. A high CHESS-AI value increases the need for health services and is associated with higher mortality.
The CHESS-AI score is designed to identify clients at high risk of rapid health deterioration.
CHESS-AI estimates instability based on care notes recorded about the client, where 0 = No instability 1 = Minor instability 2 = Low instability 3 = Moderate instability 4 = High instability 5 = Very high instability
The assessment considers
- rise in ADL-AI score within the previous 90 days
- rise in CPS-AI score within the previous 90 days
- vomiting
- limb swelling
- dehydration
- breathing difficulties
- deviations in eating
CHESS-AI values are usually close to CHESS scores (see https://pubmed.ncbi.nlm.nih.gov/12534853/), but may differ in some cases because CHESS-AI relies on daily care notes interpreted by artificial intelligence.
